Referral Partners: Refer a Client to Northwoods Haven Recovery
Outpatient addiction treatment referral partners in Minneapolis get a review of the records, a plain answer on fit, and a client who starts programming right after intake.
Northwoods Haven Recovery accepts referrals from counties, courts and probation, hospitals and detox units, residential programs, therapists, employee assistance programs, and primary care across Minneapolis and the Twin Cities. We provide two levels of outpatient care in Hopkins: an Intensive Outpatient Program and a Relapse Prevention Program, both with co-occurring mental health treatment built in.
Send a current comprehensive assessment and the records you have. We review them, tell you plainly whether the client fits, and if they do, complete intake so programming starts right after. If they need a higher level of care first, we say so.

Who refers here, and why
If Your Center Has a Client That Would Benefit From Our Programs, Let's Connect
Most referrals to Northwoods Haven Recovery come from professionals who have a client ready for outpatient care and want a program that will take the handoff seriously. Residential and detox programs discharging adults who need structure after they go home are a core fit; so are courts and probation officers with a client whose assessment recommends outpatient treatment, hospitals and emergency departments following up on a substance-related visit, and therapists, primary care providers, and EAPs who have identified a substance use disorder and need a treatment partner.

Outpatient continuity for clients who are ready for it
We treat adults 18 and older with alcohol, opioid, stimulant, benzodiazepine, cannabis, and other substance use disorders, and we treat co-occurring trauma and PTSD, depression, anxiety, bipolar disorder, and ADHD alongside them in the same programming. We do not provide detox, inpatient, residential, or medical services, and nobody here prescribes. Clients on buprenorphine, methadone, naltrexone, or psychiatric medication from their own prescriber are welcome, and the plan accounts for it.
What to send with a referral
- Comprehensive assessment, current and complete (required; if the client does not have one, we complete it here)
- Treatment plan reviews from the current or most recent episode of care
- Discharge summary, for clients stepping down from residential, detox, or a hospital stay
- Medication list, including anything prescribed for opioid use disorder or a mental health condition
- Insurance information, if applicable, so benefits can be verified before the client arrives
- Release of information signed by the client, naming who we may speak with and about what
Missing something? Send what you have and call. A referral does not wait on a complete packet; the assessment is the one document we need before a fit decision, and if the client does not have a current one, we schedule it here. We confirm the method for sending records on the first call.
How it works
Three steps from your referral to the client's first group
Referring professionals need a clear answer and a clean handoff. The process is built to give you both.
01
Send the assessment and records
Call to introduce the client, then send the comprehensive assessment, treatment plan reviews, discharge summary, medication list, insurance information, and the signed release. We tell you on that call how to send them in a way that protects the client’s privacy.
02
We review and respond
A licensed alcohol and drug counselor reviews the records and we verify insurance. You get a plain answer: the client fits, the client fits after a higher level of care, or a different program would serve them better, with a name to call if so.
03
Intake, then programming starts right after
If the client fits, we schedule intake, confirm the schedule with the client, and programming starts right after intake. With the client’s consent, you hear from us when they start and as treatment progresses.
Not sure it is a fit?
Call. We say so plainly, and we help find the right level.
If you are unsure whether a client needs residential care first, or whether the Relapse Prevention Program is enough structure, describe the situation and we give you an honest read. When the answer is a higher level of care, we help you find it and take the step-down when the client is ready.
Referring for an assessment only? How the chemical use assessment works
Why professionals refer here
A handoff should not be a hope. It should be a plan.
Professionals refer here because the client they send is the client we treat, and because we tell them when that is not the case.
Continuity after residential
Step-downs from residential and detox are a core fit. The same counselors carry a client from IOP into the Relapse Prevention Program, so the plan you built does not restart from zero.
Co-occurring care built in
Trauma and PTSD, depression, anxiety, bipolar disorder, and ADHD are treated in the same programming as substance use, by the same team, rather than referred out.
Medicaid and commercial networks
In-network with Minnesota Medicaid, Blue Cross Blue Shield, HealthPartners, UnitedHealthcare, and Medica. Benefits are verified before the client arrives.
Honest fit decisions
You hear yes, not yet, or not here, with the reasoning. Nobody is admitted to fill a group when the assessment points to a higher level of care.
Family involvement
When the client wants it, family is part of treatment. Half of the calls we receive come from a family member, and the program is built for that reality.
Communication back to you
With the client’s written consent, you hear when they start, how they are progressing, and when they complete, in the form your role requires.
What we will tell you
What we will say, and what we will not do
We will tell you whether the client fits, and why. When an assessment supports outpatient care, we say so and move to intake. When it does not, we tell you which level of care it does support and help you find it. When a client is in the Relapse Prevention Program range rather than IOP, or the reverse, we say that too. Referring professionals get the same honesty clients do, because a placement that does not fit costs everyone, most of all the client.
We will not accept a client who needs detox or residential treatment first. Northwoods Haven Recovery is outpatient only, without medical staff or prescribing, and a client in active withdrawal from alcohol or benzodiazepines, or who cannot stay safe between sessions, belongs somewhere else until they are stable. Send them anyway when they step down. The door is open, and the records you already gathered will not have to be gathered twice.
- Fit decisions come from a licensed alcohol and drug counselor's review, not a sales conversation
- Records are used and released only within the client's signed release
- A client we cannot take leaves with a referral to a program that can
Where your client lands
Two levels of outpatient care, one team across both
Every program at Northwoods Haven Recovery is outpatient. The assessment places the client in the level that fits, and most move from the first to the second with the same counselors.
Intensive Outpatient Program (IOP)
Three-hour groups most programming days, individual counseling with the client’s own licensed alcohol and drug counselor, and co-occurring mental health treatment. About two to four months.
Relapse Prevention Program
Ten to twelve weeks of low-intensity outpatient care with the same counselors, ending with a written relapse prevention plan. A fit for clients completing IOP or stepping down from residential with stable recovery.
Conditions we treat
Alcohol, opioids, methamphetamine, cocaine, benzodiazepines, marijuana, prescription drugs, and more, always through the same outpatient path.
Co-occurring mental health care
Trauma and PTSD, depression, anxiety, bipolar disorder, and ADHD treated alongside substance use in the same programming, not through a separate referral.

Therapies
Evidence-based therapies we use, taught as options
Seven approaches, one plan built with you. Your counselor teaches you what each one does and you decide what belongs in your recovery lifestyle.
Catch the thought that comes before the drink or the drug, test it, and practice a different move before Friday.
Skills for intense emotion, urges, and relationships: mindfulness, distress tolerance, emotion regulation, interpersonal effectiveness.
Make room for the craving or the feeling without acting on it, and build a life around what you value.
Find your own reasons to change, at your own pace. Includes motivational enhancement therapy, the structured short form.
The same people each session, so accountability is real. Where most of the traction in outpatient treatment comes from.
Every group and session run on predictability, choice, and safety, because most people carry something older than the substance.
Mental health, relationships, work, and purpose treated as one picture, with a recovery lifestyle you define.
You do not choose a therapy from a menu. Your counselor teaches you all of them as options and the plan uses what works for you.

Common questions
Questions referring professionals ask
Every referral is a conversation, and the specifics get settled on the first call. These are the questions counties, courts, hospitals, residential programs, and clinicians ask most.
Who can refer a client to Northwoods Haven Recovery?
Any professional working with an adult who may need outpatient substance use treatment: counties, courts and probation, hospitals and detox units, residential programs, therapists and psychiatrists, employee assistance programs, and primary care. Family members and clients themselves can also call directly; no professional referral is required.
What documents are required?
A current comprehensive assessment is required, along with a release of information signed by the client. Treatment plan reviews, a discharge summary, a medication list, and insurance information make the review faster and the handoff cleaner. If the client has no current assessment, we complete one here.
How do I send records?
Call first to introduce the client. On that call we confirm a confidential method for sending records that works for your organization and ours. Records are handled and released only within the limits of the client’s signed release.
How quickly will I hear back?
We review records and verify insurance, then call you with a plain answer on fit. We do not promise a turnaround, because the review depends on the records and the insurance response, but you will not be left guessing; if something is holding up the decision, we tell you what it is.
Will you take a client who still needs detox or residential care?
Not until they are stable. Northwoods Haven Recovery is outpatient only, with no medical staff or prescribing. A client in active alcohol or benzodiazepine withdrawal, or who cannot stay safe between sessions, needs a higher level of care first. We help find it and take the step-down after.
Do you accept clients on buprenorphine, methadone, or naltrexone?
Yes. Medication for opioid use disorder prescribed by the client’s own provider is welcome, and so is psychiatric medication. Include it on the medication list so the treatment plan accounts for it. We do not prescribe or manage medication here.
Will I get updates on my client's progress?
With the client’s written consent, yes. The release names who we may speak with and about what, and we communicate within it: start date, attendance and progress where the release allows, and completion. Court and probation reporting follows the same rule.
Which insurance plans do you accept?
We are in-network with Minnesota Medicaid, Blue Cross Blue Shield, HealthPartners, UnitedHealthcare, and Medica. Send the client’s insurance information with the referral and we verify benefits before the client arrives, so neither of you is surprised later.
