Relapse Prevention Program in Minneapolis, Minnesota
Low-intensity outpatient care for people who have done the hard work and want to keep what they built.
Northwoods Haven Recovery’s Relapse Prevention Program is the step-down level of outpatient addiction treatment, clinically called low-intensity outpatient, for adults across Minneapolis and the greater Twin Cities. It runs about ten to twelve weeks with the same counselors you already know: fewer hours than IOP, the same honesty, and a plan for the moments that test you.

Who this program is for
The step after IOP, or after care elsewhere.
Most people arrive here after finishing our Intensive Outpatient Program. Some start here after a higher level of care elsewhere. Either way, you go home after every session.

What the program is
What relapse prevention looks like as a level of care
Relapse prevention is a clinical skill set and a level of care. As a skill, it is learning to name your triggers, notice the thinking that precedes a slip, and act on a plan before it becomes one. As a level of care, our Relapse Prevention Program is low-intensity outpatient treatment: a lighter schedule for people who no longer need the structure of IOP but are not ready to walk out the door with nothing behind them.
Inside the program: group therapy with people at the same stage, individual counseling with your own chemical dependency counselor, continued treatment for co-occurring mental health conditions, and aftercare planning that turns into a written plan you leave with. The work shifts from stabilizing to practicing: the job, the relationships, the weekend, the anniversary, handled with your counselor still in the picture.
Recovery is your recovery lifestyle, not ours. Northwoods Haven Recovery is inclusive of every belief system and every recovery approach, so the plan you leave with is one you actually chose. We rise together.
One thing to be clear about
We are outpatient only. No detox, no inpatient, no residential care. If you are in active withdrawal, that needs medical attention first, and we will help you find it.
Our space
Inside our outpatient treatment center in Hopkins, Minnesota
A safe haven should feel like one. Our suite on Green Oak Drive looks out over water and trees, and the rooms where groups meet were built to feel calm, comfortable, and homey. Not clinical, not institutional. You are more than your diagnosis here, and the space is meant to say so before anyone does.
The lounge
Where people land and catch their breath before group.
The group room
Three hours a day, in a circle, with people who understand.
The view
Water and treeline out every window: the northwoods, in the metro.
Another Group Room
Three hours a day, in a circle, with people who understand.
Quiet corners
Somewhere to sit with a thing before you say it out loud.
Room to breathe
Daylight and treeline out every window. No clinical fluorescence.
Programming
What a week in the Relapse Prevention Program looks like
Your schedule is individualized and lighter than IOP. This is the shape most weeks take.
Group therapy
Fewer sessions
Group sessions with people at the same stage of recovery, scheduled around work and family rather than in place of them.
Individual counseling
As planned
With the same chemical dependency counselor you worked with in IOP, when your plan calls for it.
Relapse prevention planning
Built in
Triggers named, warning signs mapped, and a written plan that gets revisited as life happens.
Everything else
Yours
This level of care exists so most of your week is your life, with support still in reach.
Who it is for
Who the Relapse Prevention Program is right for
- You have finished our Intensive Outpatient Program and want to keep the momentum without the same time commitment
- You completed residential, inpatient, or another program elsewhere and are stable, but want structured support as you return to daily life
- You have had a slip or a close call and want to strengthen your plan before it becomes a pattern
- You are managing a co-occurring mental health condition and want that care to continue alongside your recovery
- You want to keep a counselor and a community in your corner while you rebuild routines, relationships, and work
Not sure yet
A comprehensive assessment answers it.
A chemical use assessment determines whether the Relapse Prevention Program is the right level of care or whether IOP is a better starting point. If you already have an assessment or a discharge summary from another program, send it over and we will review it instead of repeating it.
For families
About half our calls are from someone else.
You can ask a question without committing anyone to anything, and family involvement is part of the work when the client wants it.
Levels of care
The Relapse Prevention Program is the step-down, not the end
Most people move through both of our levels of care. Aftercare planning runs through every level rather than being sold separately.

Relapse prevention and aftercare planning run through every level of care rather than sitting off to the side, and whole-person care means mental health psychoeducation happens alongside all of it.
Substance use
Substance use disorders we treat in Minnesota
Addiction is a disease, never a moral failing, and you are more than your diagnosis. Northwoods Haven Recovery treats alcohol and drug use disorders in an outpatient setting, with the same person-centered approach no matter what brought you here. This isn’t a complete list. If what you’re struggling with isn’t here, call us. We’ll tell you honestly whether we’re the right fit.
Mental health
New
Co-occurring mental health conditions we treat alongside addiction
Substance use and mental health rarely arrive separately. Northwoods Haven Recovery now treats co-occurring mental health conditions alongside chemical dependency programming, so you don’t have to choose which one to work on first. If your needs are mental-health-primary rather than co-occurring with a substance use diagnosis, a program built for that will serve you better, and we’ll help you find one.
Trauma and PTSD
Trauma-informed care that accounts for what happened to you while you treat substance use.
Depression
Support for low mood, hopelessness, and the drinking or use that often travels with them.
Anxiety
Skills for panic, worry, and the urge to self-medicate, taught alongside your program.
Bipolar disorder
Care that steadies mood episodes while chemical dependency programming continues.
ADHD
Structure and skills for adult ADHD when stimulants, alcohol, or cannabis have become the coping strategy.

Common questions
Questions people ask about relapse prevention
If yours is not here, ask us directly. We would rather tell you honestly that we are not the right fit than have you find out later.
What is a relapse prevention program?
At Northwoods Haven Recovery it is our step-down level of outpatient care, clinically called low-intensity outpatient. It runs about ten to twelve weeks with group therapy, individual counseling, continued co-occurring mental health treatment, and a written relapse prevention plan. It is designed for people who have finished IOP or are stable and want structured support.
Is this the same as low-intensity outpatient treatment?
Yes. Low-intensity outpatient is the Minnesota clinical term for this level of care. We call it the Relapse Prevention Program because that is what the work is about at this stage.
Do I have to complete IOP first?
Not always. Most people step down into this program from our Intensive Outpatient Program, but if your chemical use assessment shows you are stable and need less structure, or you recently completed a higher level of care elsewhere, this can be the right starting point.
How long does the Relapse Prevention Program last?
About ten to twelve weeks, individualized to you. Your plan is reviewed as you go, so the timeline reflects your progress rather than a fixed package.
What if I relapse during the program?
You call us. A slip is information, not a verdict, and it is exactly what the plan is for. Depending on what happened, you may continue in the program, step back up to IOP for a time, or need a higher level of care, and we will tell you honestly which.
Can I keep working while I am in the program?
Yes. This level of care asks for fewer hours than IOP and is scheduled around work, school, and family. Most people are back to a full routine while they are here.
Does the program treat mental health conditions too?
Yes. Co-occurring mental health treatment continues at this level, and mental health psychoeducation runs through the whole program.
Does Northwoods Haven Recovery provide detox or inpatient care?
No. We are outpatient only. If you are in active withdrawal or need residential care, call us anyway and we will help you find a provider, then pick up your care when you are ready.
