Rehab That Takes UnitedHealthcare in Minneapolis
Outpatient addiction and mental health treatment in-network with UnitedHealthcare, for adults across Minneapolis and the Twin Cities, whether your card says UnitedHealthcare or Optum.
Northwoods Haven Recovery is in-network with UnitedHealthcare, one of the largest health plans in the country and one that many Twin Cities employers offer. UnitedHealthcare plans, whether through work or bought individually, include behavioral health coverage for substance use disorder and mental health treatment. When a comprehensive assessment shows that outpatient care is the right level, the Intensive Outpatient Program and Relapse Prevention Program at our Hopkins offices fall within that coverage.
With UnitedHealthcare, the behavioral health benefit is often administered by Optum, so your card may say UnitedHealthcare on the front and list Optum for mental health on the back. Tell us what is on the card and we handle the rest. We verify your benefits, explain them plainly, and tell you what you would owe before you commit.

How UnitedHealthcare works here
A national plan, a local program, and one call to connect them
UnitedHealthcare is a national insurer, and in Minnesota most of its members are covered through an employer, with a smaller number on individual plans. Because it operates everywhere, its plans are designed to be used everywhere, and an outpatient program in Hopkins is an ordinary in-network claim for a Twin Cities member. Substance use and mental health treatment sit within the behavioral health portion of the plan. For many UnitedHealthcare members that portion is administered by Optum, a company in the same corporate family, which is why the mental health phone number on the back of the card is often different from the medical one.
None of that changes the basic path. Coverage for a level of care follows medical necessity, and medical necessity is established by the comprehensive chemical use assessment everyone at Northwoods Haven Recovery completes with a licensed alcohol and drug counselor. We contact whichever number your card gives for behavioral health, confirm your eligibility, ask whether prior authorization is needed for outpatient treatment, and submit the assessment when it supports that level of care. If it instead shows you need detox or residential treatment first, your plan can cover that with a provider who offers it, and we help you find one and welcome you back after.
Card says Optum, or a different behavioral health number? That is common with UnitedHealthcare and it does not mean anything is wrong with your coverage. Optum often administers the behavioral health benefit on the plan’s behalf. Read us exactly what the card says, front and back, and we call the right place and come back to you with a plain answer.
How it works
Three steps from your first call to coverage you understand
Nobody starts treatment here without knowing what their plan covers. With UnitedHealthcare, the first step includes finding the right number to call, and that is on us.
01
You call and we verify
Read us your UnitedHealthcare card. We contact the behavioral health administrator it lists, confirm your eligibility and benefits, and check whether your plan requires prior authorization for outpatient care. Then we tell you what we found.
02
You complete the assessment
A comprehensive chemical use assessment with a licensed alcohol and drug counselor establishes your diagnosis, the level of care that fits, and the medical necessity your plan relies on to approve treatment.
03
Treatment begins, covered
When outpatient care is recommended and approved, programming starts right after intake. We manage authorizations and progress reviews with your plan’s administrator while you focus on treatment.
Wondering what your share will be?
Ask first. It is the question we expect and the one we answer most carefully.
UnitedHealthcare plans vary widely from one employer to the next. We look at yours, explain your deductible, copay, or coinsurance in ordinary words, and give you the full picture before you decide anything.
Questions first? How admissions works
Why here
In-network with UnitedHealthcare, and clear about what that means for you
A big national plan can feel impersonal. The program on the other end of it does not have to.
In-network with UnitedHealthcare
Contracted rates and your in-network cost sharing apply, which is the most affordable way to use the coverage you already have through work or on your own.
We call the right number
UnitedHealthcare on the front, Optum on the back, or something else entirely: we figure out who administers your benefit and reach them. You make one call.
Coverage confirmed before you start
You know what your plan covers and what your share would be before your first session, not when an explanation of benefits arrives.
One assessment, two purposes
The comprehensive assessment your plan needs for authorization is the same one that decides your level of care. You do it once.
Mental health inside the program
Co-occurring anxiety, depression, trauma, bipolar disorder, and ADHD are treated with the same counselors, under the same behavioral health benefit.
Honest about fit
If your assessment shows you need detox or residential care first, we say so and help you find an in-network provider who offers it. You are welcome back after.
What insurance decides, and what it does not
UnitedHealthcare approves the level of care. The rest is yours.
Your plan, or the administrator that handles its behavioral health benefit, authorizes treatment based on medical necessity and may review progress at intervals. That is routine, and our team handles the paperwork. You will hear three terms when we walk through your plan. A deductible is the amount you pay before the plan starts paying. A copay is a fixed amount you pay for a visit. Coinsurance is the percentage of a cost you and the plan share. We explain which of these apply to outpatient treatment on your UnitedHealthcare plan and what they mean for you.
What the plan does not decide is how you work your recovery, what you believe, or what your life looks like afterward. That stays yours. And when there is a limit worth knowing about, a pending authorization, a benefit that resets with the plan year, or a specific UnitedHealthcare product we are not contracted for, we tell you as soon as we know rather than letting you find out from a bill.
- UnitedHealthcare benefits verified on the first call, through whichever administrator your card lists
- Prior authorizations and progress reviews handled by our team
- Your share explained in plain language before you begin
What UnitedHealthcare covers here
Two levels of outpatient care, and the conditions treated inside them
Everything at Northwoods Haven Recovery is outpatient, and both levels of care are billed to your UnitedHealthcare plan, through its behavioral health administrator, when your assessment supports them.
Intensive Outpatient Program (IOP)
Three-hour groups most programming days, individual counseling with your own licensed alcohol and drug counselor, and co-occurring mental health care. About two to four months.
Relapse Prevention Program
Ten to twelve weeks of lower-intensity outpatient care with the same counselors after IOP, built around a written relapse prevention plan.
Conditions we treat
Alcohol, opioids and fentanyl, methamphetamine, cocaine, benzodiazepines, marijuana, and other substances, all on the same outpatient path.
Co-occurring mental health care
Anxiety, depression, trauma and PTSD, bipolar disorder, and ADHD treated alongside substance use, by the same team.
Substance use
Substance use disorders we treat in Minnesota
Seven groups, one outpatient path. The substance shapes the withdrawal risk and the early weeks, not the level of care or the counselors you work with.
The most common reason people call us, and the one most often hidden behind a functioning life.
Prescription opioids, heroin, and the fentanyl that now contaminates most of the illicit supply. Detox often comes first. We do not provide it, but we help you find it.
Methamphetamine, cocaine, and prescription stimulants. Group work is where most of the traction comes from.
Benzodiazepines, sleep medications, and other prescriptions that started as treatment. Stopping abruptly can be dangerous, so a medical provider guides the taper.
Legal does not mean harmless. Cannabis use disorder is a real diagnosis.
LSD, ecstasy and MDMA, ketamine, and PCP. Usually part of a wider pattern, which is what the assessment sorts out.
Inhalants, nitrous oxide, and substances that show up faster than the research does.
Not on the list?
This list is not a filter. Call us and we will tell you honestly whether we are the right fit, and help you find who is if we are not.
Mental health
Co-occurring mental health conditions we treat alongside addiction
Treating one and ignoring the other does not work. Northwoods Haven Recovery treats co-occurring mental health conditions alongside chemical dependency programming. If your needs are mental-health-primary, a program built for that will serve you better, and we will help you find one.
Trauma and PTSD
Care that accounts for what happened to you, not what is wrong with you.
Depression
The loop between using and feeling worse, treated from both ends at once.
Anxiety
Using to quiet it works for an hour and costs more than that.
Bipolar disorder
Care that steadies mood episodes while chemical dependency programming continues.
ADHD
Structure and skills for adult ADHD when stimulants, alcohol, or cannabis became the coping strategy.
Something else?
If a mental health condition is tangled with substance use, tell us. We treat both together or help you find who can.

Common questions
Questions about using UnitedHealthcare for addiction treatment
UnitedHealthcare plans differ by employer and product, so the first call is where your specific answer comes from. These are the questions we hear most.
Does Northwoods Haven Recovery accept UnitedHealthcare?
Yes. We are in-network with UnitedHealthcare. Give us the information on your card during the first call and we verify your benefits, through whichever administrator the card lists, before anything is scheduled.
Does UnitedHealthcare cover outpatient rehab in Minnesota?
Yes. Substance use disorder treatment, including outpatient levels of care such as the Intensive Outpatient Program and the Relapse Prevention Program, is part of the behavioral health benefit on UnitedHealthcare plans when a comprehensive assessment shows it is medically necessary.
My card says Optum for mental health. Does that change anything?
Not for you. Optum often administers behavioral health benefits for UnitedHealthcare members, so it is the number we call to verify coverage and request authorization. Tell us what the card says and we handle the rest.
What is the difference between in-network and out-of-network?
In-network means the provider has a contract with your plan, so negotiated rates and lower in-network cost sharing apply. Out-of-network providers have no contract, and your share is usually much higher or the care may not be covered. Northwoods Haven Recovery is in-network with UnitedHealthcare.
Does UnitedHealthcare require prior authorization for outpatient treatment?
Some plans do, and the comprehensive assessment is what supports the request. We find out during the first call, handle the paperwork with your plan’s administrator, and tell you where things stand so you know your timeline.
Does UnitedHealthcare cover mental health treatment along with addiction?
Yes. Co-occurring conditions such as anxiety, depression, trauma, bipolar disorder, and ADHD are treated inside our outpatient programming, and that programming is what your plan’s behavioral health benefit covers.
Does UnitedHealthcare cover detox or residential treatment?
It can, through providers who offer those levels of care. Northwoods Haven Recovery is outpatient only. If your assessment shows you need detox or residential care first, we help you find an in-network provider and welcome you back afterward.
What if I do not have UnitedHealthcare, or any insurance?
Call anyway. We are also in-network with Minnesota Medicaid, Blue Cross Blue Shield, HealthPartners, and Medica, and if you have no coverage we tell you honestly what your options are, including whether a public program may be open to you, and help you find the right next step.
